Freebie! Primary Care Follow Up Visit Note Outline

If you have followed me on Instagram or read any of my blogs, you probably know that I believe having notes for your doctor's visit is super important. When I first became sick I had no idea how to make effective notes for doctor's visits. I often would just have a quickly written list of my symptoms that 90% of the time I didn't even bother to pull out. As the years have gone by, I have figured out more effective ways to organize my notes. As I was preparing for an appointment with my Primary doctor, I decided to create a template to share! So here is my PCP Follow Up Notes template.

I recommend making two copies when using it, one for you and one for your doctor. It also helps if you can give it to your doctor's nurse as you are being roomed, that way he/she has a chance to read over it.
